New Patient Registration
If you would like to register as a new patient please first check whether your live within the relevant catchment area. Then please visit the surgery to collect a registration pack which contains instructions and all the forms needed to register.
Or if you prefer, you can print out the Registration Form, New Patient Questionnaire and Your Data Sharing Preferences below. Please hand in your completed forms to reception together with some form of photographic ID (eg driving licence or passport). We are not able to register patients until we have received ALL completed forms.
To allow us to provide you with as much help as you might need, please avoid busy periods wherever possible when returning your paperwork.
When you register, if you are over 16, we as that you provide a urine sample which you will need to give us before 11:00.

We aim to register new patients within 5 days of receipt of all completed paperwork. If you think you will need to be seen before this, please speak to reception when you hand in your form.

Nominating a friend or relative to have access to your medical information
You may wish to consider whether you would like to nominate a friend or relative to have access to your medical information. This make particularly be beneficial for those patients who receive help from a carer or who find it difficult to manage their own affairs. It means that your friend or relative can speak to the surgery on your behalf regarding your care.
If you would like to nominate someone to access your medical records, please print and complete this form and return it to the surgery once completed. Forms are also available from reception.
